What to Bring to your EOC Appointment or Workshop

The following is a general list of documents that you should bring to your scheduled EOC appointment or workshop:

  • Your Social Security Number;
      **Use your legal name as it appears on your Social Security card. Using a nickname or any other name will cause a processing delay.

      **If you have recently married and use your married name on the FAFSA, you must also have submitted the name change with the Social Security Administration. Failure to do so will significantly delay the financial aid process.

  • Your 2008 Year Tax Return (for the 2009-2010 FAFSA) and/or 2009 Year Tax Return (for the 2010-2011 FAFSA);
      **If you are unable to locate your or parent(s)’ tax information, please complete a IRS 4506-T Form.

  • Parent(s)’ 2008 Tax Return (for the 2009-20109 FAFSA) and/or 2009 Year Tax Return (for the 2010-2011 FAFSA) if you are a Dependent Student;

  • Dependent Pre-Application Worksheet;

  • Documentation of untaxed financial assistance such as Social Security Disability benefits and/or child support;

  • List of continuing education programs, colleges, and/or universities that you have an interest to enroll/ apply.

    Please note the 2009-2010 FAFSA provides aid for the Spring 2010 and Summer 2010 semesters and the 2010-2011 FAFSA provides aid for the Fall 2010, Spring 2011 and Summer 2011 semesters.
