Requesting Disability Accommodations

You may open and print the forms required for requesting disability accommodations through MTC's Counseling & Career Services office using the applicable links below.

(Note: Adobe Acrobat Reader is required for all forms)

  1. Complete a Disability Services Intake Form and a SC Voter Registration Declination Form:

    If you would like to request accommodations for taking MTC's placement test (i.e., the COMPASS), you will also need to complete a Placement Test (COMPASS) Accommodations Form.

    You will need to submit acceptable documentation from a professional qualified to diagnose your disability.

  2. Depending upon your disability/diagnosis, open and print the most applicable form from the standardized documentation options below. Ask your licensed medical care provider to complete the applicable form. (Generally, medical care providers may provide supporting documentation in a memorandum on official letterhead, in place of completing a standardized documentation form.)

    Carefully read the “Documentation Guidelines” on the “Disability Services Intake Form” (referenced above) to determine if you may already have convenient access to the appropriate documentation required to support your request for accommodations. Please note that students seeking accommodations for a learning disability will be required to submit a current psychological-educational report showing the standard test scores used in making the learning disability diagnosis; generally an IEP from high school is not adequate documentation for a learning disability. Contact MTC’s Counseling & Career Services if you have questions concerning acceptable documentation.

  3. Return the completed Disability Services Intake Form, Voter Registration Declination Form, and appropriate supporting documentation (see #2 above), plus the Placement Test Accommodations Form (only if requesting accommodations for MTC’s placement test) to MTC’s Counseling & Career Services office – in person, by postal mail, by fax, or by scanning the documents and sending them as e-mail attachments. Before submitting the forms/documents, please confirm that all requested information is complete and accurate.

  4. Counseling and Career Services staff will review your intake packet and documentation and will contact you via phone, e-mail, and/or letter informing you of the status of your request.

  5. You must inform Counseling Services each semester of your accommodation needs in each class for which you are registered. We ask that you make your requests at least 30 working days prior to the first day of classes.
