         Based on the pleadings, financial declaration, sworn testimony, and other evidence of record, I make the following Findings of Fact and Conclusions of Law:

         The Court has jurisdiction over the parties and the subject matter of this action and the venue in Lexington County is proper.

1.              Petitioner(s) is a resident of ();

5.              The Guardian ad Litem of the child/ren consents to this adoption:

6.              Petitioner(s) is/are capable of bring up the child/ren and of meeting the food, clothing, shelter, medical, emotional and educational needs; and the Petitioner(s) is/are a fit and proper person(s) and (he,she,they) desires to establish the relationship of parent(s) and child/ren;

         7.       The minor child/ren has/have been in the actual custody of the Petitioner(s) for more than ninety  (90) days;

         8.       All necessary consents or relinquishments for the purpose of adoption have been obtained;

         9.       Notice of the adoption proceedings have been given to all persons entitled to notice under Sections 20-7-1690 and 20-7-1734 South Carolina Code of Laws, and all necessary hearings have been held;

10.           The natural mother/father of the child consents to the adoption;

11.           The rights of the natural father of the child was terminated by the Court;

12.           Notice of the adoption proceedings have been given to all persons entitled to notice under 20-7-1690 and 20-7-1734, and all necessary hearings have been held;

13.           The natural mother/father waived her rights to be notified of any hearings of said minor child;

14.           The natural mother/father of the minor child/ren has/have not supported said child/ren for more than six months prior to the commencement of this action;

15.           The natural mother/father of said child/ren has/have not visited the child/ren for more than six months prior to the commencement of this action;

16.           The Defendant/Defendants was/were served on ______________ and is/are in default as evidenced by an Affidavit of Default filed with the Court at the time of the hearing;

17.           The Petitioner desire the child name to be changed to ________________;

18.           The best interest if the said minor child/ren would be served by wavier of the adoption pre-investigation and post-investigation because __________

19            The Guardian ad Litem for the minor child recommends that __________

         20.     An adoption investigation was completed pursuant to the South Carolina Code of Laws and the investigator finds the Petitioner satisfactory;

         16.     The disbursements made and accounted for pursuant to Section 20-7-1775 South Carolina Code of Laws are reasonable cost for expenses incurred or for fees for services rendered;

17.           It is in the best interest of the minor child that this adoption be allowed;

IT IS THEREFORE ORDERED:

1.              The relationship of parent and child between _________________ and the minor  child is hereby created.

         2.       The parental rights of _______________ are terminated and this adoption is allowed without his consent.

3.            The minor child’s name be changed to ______________.

4.             This be taken as a final Decree of Adoption and the record is required by

statute be sealed and made available only upon Order of this Court.
